    Following a discovery that a member of staff had disclosed information about my care I made a complaint to their advice and complaints department, following a number of months I received a report, this identified that the nurse who at no time was involved in my care had accessed my medical records including very sensitive information on 3 occasions for a total of 31 minutes , however their report played down the seriousness accepting the nurses excuse that the first access was to check my date of birth, the second for my phone number and most unbelievable of all the third she claimed could not remember but believes she must have caught my record (yes they thought that was possible). They claim the report was impartial , however within 7 days of the incident and before the investigation they completed an incident report stating 'No Harm Done' and within 21 days of this the nurse had completed her disciplinary (a learning exercise) this before the investigation was complete and the report compiled. Following the report I made 2 GDPR requests which they initially refused to give me, however pressure from the ICO forced them to comply, this information showed that the trust had not only played down the actions of the nurse but they had also altered fact and where the facts could not be made match the nurses story, they simple omitted them from the report, 2 of the most serious were: Accepting the nurses claim that the first access was because she noticed my name and entered the record to check my date of birth, then left the record open for 21 minutes as she'd contacted a colleague for advice, however their own evidence from this second nurse clearly show the conversation took place over 1 hour before the nurse accessed my record at 19:01 on a Saturday evening on a laptop provided to her by the trust, therefore she was fully aware of who I was and as the second nurse informed her of the care I was receiving, what I was been treated for, in addition as the telephone call did not take place during the access of my record, clearly she was reading it. The second cover up the investigators invented a Nurse 3 to hide the fact that the nurse had disclosed information to a friend, and I have email audits that show that not only did they not know what she disclosed, but when this was pointed out to them before the report was released, rather than correct the statement, they added a response from nurse 3 And throughout all this the trust have offered or provided no support to me Please feel free to contact me I'm happy to provide the evidence to support this statement

    I am a former service user with Bradford District Care NHS Foundation Trust but now just go to my local GP practice and get support from separate services such as Mind in in Bradford etc

    These nurses are unhelpful and its like they think your something come off they shoe. I asked them to do something for my brother whoo is in a wheelchair they lied and fibbed me off by saying they do it but never did. They speak rude tomorrow uou and are very in helpful
