Needs to always log information accurately about a service user or their immediate family on medical records. If the subject of a conversation is not present during a consultation to confirm or deny an allegation, then the doctor should include the words alleged or allegedly or put the allegation in inverted commas. Furthermore, the doctor should alert social services of any allegation concerning child welfare on the very day it was told to her. Additionally, she should write a report within forty-eight hours stating the allegation and what action she took to keep the child safe. This report should then be submitted to the relevant authorities.
